Few states regulate patient doses from diagnostic x-rays. The state of Illinois has, however, successfully specified maximum patient doses for routine radiographic examinations which has greatly improved patient protection and improved radiographic quality. Medical radiographs are limited to a maximum of 1400 mR per radiograph, and it is recommended that the measured value at the tabletop for each radiographic exposure not exceed 100 mR. Illinois has also limited routine intraoral radiography to an incident exposure of 100 mR per radiograph and a limit of 500 mR per examination. A national conference on referral criteria for x-ray examinations was held in 1978 by the Bureau of Radiological Health (now the National Center for Devices and Radiological Health) of the US Public Health Service. The conference affirmed President Carter s Federal Radiation Guidance by identifying major factors causing excessive radiological exposure: excessive radiation per film; excessive films per examination; excessive examinations per patient; lack of radiological screening of requests; and defensive medicine. Retakes because of faulty films were found to require about 27 million additional films per year. It was suggested that routine skull examinations for patients with headache or vertigo without other physical signs could be limited to a single film for screening. A periodic upper gastrointestinal series for patients with healed duodenal ulcers was found to be generally unnecessary because the patient s response could usually be determined by the clinical course observed; and similar findings were also made for routine screening x-rays, except for high-risk groups. Defensive medicine, a consequence of existing malpractice mores, was believed to comprise a significant percentage of the millions of x-ray examinations given each year.

Telemedicine and teleradiology are becoming increasingly important as the U.S. healthcare delivery system gradually changes from fee-for-service to managed capitated care. Over the past ten years we have seen a trend of primary care physicians joining health maintenance organizations (HMOs). HMOs purchase smaller hospitals and form hospital groups under the umbrella of HMOs. Also academic institutions form consortia to compete with other local hospitals and HMOs. This consolidation allows the elimination of duplication and the streamlining of healthcare services among hospitals. As a result costs are reduced, but at the same time because of the downsizing, the number of experts available for service decreases. Utilization of telemedicine and teleradiology is a cost-saving method that also compensates for the loss of expertise. One of the most comprehensive reviews in assessing telecommunications in healthcare was a study reported by the Institute of Medicine Committee on Evaluating Clinical Applications of Telemedicine in 1996 and its subsequent revisions. Various issues in telemedicine including technical and human context, policy, and methods of evaluation are discussed in detail in this report, so it is an excellent source of reference. Telemedicine, in terms of applications, can be simply de ned as the delivery of healthcare using telecommunications, computer, and information technologies (IT) that include medical images. The well-established consultation by means of telephone conversation alone would not be quali ed as telemedicine because it only uses telecommunications, and not the computer or IT. There are two models in telemedicine that involve teleradiology: The referring physician or healthcare provider can, on one hand, consult with specialists at various locations through a
